Form 990, Part VI, Section B, line 11b The form 990 is prepared by an independent accountant. The Director of Finance then reviews and compares the information on the 990 with the audited financial statements. The form 990 is then reviewed by the CEO, finance committee and board of directors before filing. Form 990, Part VI, Section B, line 12c The policy applies to the members of the board of directors, officers, and members of senior management of the Independence Center. These individuals are required to submit a disclosure statement annually as well as update the disclosure statement throughout the year if a conflict of interest arises that has not been previously disclosed. When a conflict of interset is identified, the individual is prohibited from participating in deliberations and decisions in the transaction.
