Sixty-nine AMC service area residents completed the survey. Additional information was collected through eight key informant interviews with community members. The input from the residents, who primarily reside in McIntosh County, represented the broad interests of the communities in the service area. Together with secondary data gathered from a wide range of sources, the survey presents a snapshot of the health needs and concerns in the community. Ashley Medical Center Part V, Section B, Line 11: The top four priorities identified in our 6/30/2025 CHNA were:-Attracting and retaining young families-Having enough child daycare services-Ability to retain primary care providers and nurses-Cost of health insuranceThe hospital will not address daycare services, as it is not feasible for the organization to provide daycare services due to limited staff.Attracting and retaining young families- The facility continued to maintain and expand access to local healthcare services to support community stability and quality of life for young families. Actions included maintaining essential primary care, emergency, and ancillary services locally to reduce the need for out-of-area care. The facility also continued community outreach efforts and participation in local workforce and economic development discussions aimed at sustaining the community as a viable place for families to live and work.Ability to retain primary care providers and nurses- The facility implemented and maintained competitive compensation and benefit structures for clinical staff, including providers and nursing personnel. Recruitment and retention efforts included the use of contract providers as needed to ensure continuity of care, support for continuing education and licensure, and investment in workplace resources to reduce burnout and maintain safe staffing levels.Cost of health insurance- The facility continued to offer employer-sponsored health insurance coverage to eligible employees and dependents. Efforts were made to manage benefit costs while maintaining coverage levels, including plan design reviews and participation in group purchasing arrangements to help mitigate premium increases for employees. The facility has little impact on the cost of health insurance outside its own organization and no action was taken to decrease the cost of health insurance to the outside community.The hospital also addressed assisted living options, as that was identified as a need in the 06/30/2022 CHNA. The facility is still in the process of converting eight independent living apartments into assisted living apartments. They are currently renovating each apartment as they become vacant and hope to be able to meet this need in the near future. 1 1 - Ashley Medical Center SNF 612 Center Ave N Ashley ND 584130450 Long Term Care Facility Part I, Line 3c: The Hospital also considers medical indigency and asset level to determine eligibility for financial aid. Part I, Line 7: Charity care expense was converted to cost on line 7a based on an overall cost-to-charge ratio addressing all patient segments. Community health improvement services, line 7e, is actual costs reported in the general ledger. The cost for Medicaid on line 7b and subsidized health services reported on line 7g were determined using the cost report. Part III, Line 2: The amount on line 2 represents implicit price concessions. The Organization determines its estimate of implicit price concession based on its historical collection experience with this class of patients. Part III, Line 3: The organization reasonably estimates the portion of implicit price concessions that could have qualified for charity care based on a 20% federal poverty level proxy. An estimated 20% of implicit price concessions in 2024 are attributable to patients potentially eligible for charity care. Part III, Line 4: The financial statement footnote regarding implicit price concessions may be found on page 10-11 of the audited financial statements. Part III, Line 8: Medicare allowable costs of care are based on the Medicare cost report. The Medicare cost report is completed based on the rules and regulations set forth by Centers for Medicare & Medicaid Services. The costs themselves are a community benefit as the community would not receive these services if we did not provide them. Part III, Line 9b: It is the policy of the Ashley Medical Center to forego extraordinary collection efforts against an individual before the organization has made a reasonable effort to determine whether the individual is eligible for assistance under the hospital charity care policy. Those who qualify for financial assistance will not be charged more than the amount generally billed to other patrons for Emergency Services (described in the EMTALA) or Medically Necessary services. The hospital billings and collections policy provides written notice at least 30 days prior to initiating collection actions. The hospital does not send patient accounts to collections until at least 120 days after the date of the first invoice. If a patient applies for financial assistance within 240 days of the date of the first invoice the hospital will pull that account or invoice back from collections. Part VI, Line 2: The Ashley Medical Center conducts an annual program review to evaluate current programs as well as determine any future programs that may be needed within our healthcare community. Part VI, Line 3: Ashley Medical Center provides insurance counseling throughout the year to our patients and provides contact information for state and federal programs. We provide patients of need with a copy of our financial assistance program and post it at various locations throughout the facility including the ER, admissions, and AMC Clinic. Our financial assistance program and its summary is posted to our website. Part VI, Line 4: Ashley Medical Center (AMC) provides health care services to a rural and frontier geography that includes the cities of Ashley, Lehr, Zeeland, and Wishek in McIntosh County; Gackle, Fredonia, and Napoleon in Logan County; Ellendale and Forbes in Dickey County; Hague and Strasburg in Emmons County; and Kulm in LaMoure County, North Dakota. In South Dakota, AMC serves patients from Long Lake and the Leola area in McPherson County, as well as Eureka in Campbell County. In total, AMC serves approximately 3,500 residents across a multi-county rural region. McIntosh County, which represents AMC's core service area, has an estimated population of approximately 2,500 residents. Population density in the primary service area averages approximately 4 people per square mile, compared to roughly 90 people per square mile nationally. Geographic isolation, long travel distances, and severe winter weather conditions significantly impact access to health care. AMC strives to ensure access to high-quality care regardless of these geographic barriers. Ashley, ND is located in southcentral North Dakota, approximately 120 miles southeast of Bismarck and six miles north of the South Dakota border. Along with the hospital, the courthouse, school, and agriculture provide the economic base for the town of Ashley and McIntosh County. Ashley has a number of community assets and resources that can be mobilized to address population health improvement. The community has a number of physical assets and features, including a fitness trail, swimming pool, city park, pickleball courts, golf course, and fitness center. Lake Hoskins Park and Dry Lake offer recreation, fishing, and camping opportunities. McIntosh County offers prime hunting opportunities. Ashley offers cultural attractions with the Heritage Center, which pays tribute to the early history of the area.With regard to demographics, McIntosh County's population from 2020 to 2023 decreased by 1.9 percent. The average number of residents younger than age 18 (21.3%) for McIntosh County comes in 2.2 percentage points lower than the North Dakota average (23.5%). The percentage of residents ages 65 and older is 13.5 percent higher for McIntosh County (30.2%) than the North Dakota average (16.7%), and the rate of education is 6 percent lower for McIntosh County (87.6%) than the North Dakota average (93.5%). The median household income in McIntosh County ($64,236) is much lower than the state average for North Dakota ($78,538). McIntosh County remains predominantly White (approximately 96%), with small populations identifying as Native American, Asian, Pacific Islander, Black or African American, or multiracial. Approximately 2% of residents identify as Hispanic or Latino. Part VI, Line 5: The Ashley Medical Center (AMC) is a non-profit, community owned hospital located in Ashley, North Dakota. AMC is governed by a publicly elected nine-member board of directors. These members are residents of our community. It is the largest hospital complex in McIntosh County and serves an approximate 50-mile radius around Ashley. AMC extends medical staff privilieges to all qualified physicians in its community. Surplus funds, if any, are reinvested for facilty and equipment upgrades, as well as staff recruitment and retainment.It is the policy of the Ashley Medical Center to provide as direct services medical emergency procedures as a first response to common life-threatening injuries and acute illness. Services are available to all residents, patients, and visitors regardless of age, race, national origin, disability, or ability to pay.In Fiscal Year 2025 the Ashley Medical Center continued as a host site for Bismarck State College's CNA course. In doing this the Ashley Medical Center produced 3 licensed CNA's. AMC promotes this course by presenting it to the juniors and seniors at the Ashley Public School as well as advertising to the community through the local paper and AMC's Facebook page.We provide an annual heart check wellness. This screening is available anytime during the calendar quarter of the patient's birthday. We offer this screening at a reduced rate of $50 as opposed to the standard price of $205 to have the lab tests provided. The directors and officers are required to annually disclose any potential conflicts. The board of directors reviews for conflicts. If a conflict is identified the person must abstain from voting and remove themselves from discussions.
