Form 990, Part VI, Section B, Line 12c All members of the Board of Directors, Executive Director and Director of Finance must complete a disclosure form identifying any relationships, positions, or circumstances that the responsible person is involved in that he or she believes could contribute to a conflict of interest. Any such information regarding potential conflict of interest shall be made available to the Board President and Executive Director. The conflict-of-interest policy be reviewed annually by each member of the Board of Directors.
