Emergency room and urgent care visits were 6,062.Preventive Care Services:Edgerton Hospital and Health Services (Edgerton Hospital) provides several types of free health screenings during the year. These include: blood pressure, dietary counseling and education, diabetes, and cancer screenings.Emergency Room:Edgerton Hospital's Emergency Room is fully staffed 24 hours a day, seven days a week. Edgerton Hospital is affiliated with "Life Flight," a helicopter service that delivers patients to area tertiary care centers. Edgerton Hospital's Urgent Care Center provides another level of walk-in care when appropriate.Community Outreach:Edgerton Hospital works closely with the community to provide hospital and hospital-related services responsive to the specific needs of Edgerton and surrounding farming communities. Senior HealthAll seven needs were and currently continue to be addressed by EHHS.For Mental Health Services, we reviewed current services provided and looked to boost education for the patients and general public by hosting an educational session for the public by our mental health providers. We also reviewed patient care by the providers and re-entered patients into care as the need arose. Further, we continually look at the patient panel of our mental health providers, striving to add additional patients are their schedule permitted.For Drug/Substance Abuse, we held numerous Narcan trainings for the hospital Trustees, local residents and local school faculty and staff. In addition, we continue our work to identify additional mental health provider(s) that can treat drug and substance abuse. We take careful steps to facilitate patient referrals to local agencies who commonly deal with drug/substance abuse.For Youth Mental Health Services, we provided these services locally with both our mental health counselor in the office, mental health NP prescriber and placement of a mental health counselor in both the Milton and Edgerton School districts. In addition, our pediatric nurse practitioner sees child/adolescent patients in her practice with mental health diagnoses.For Primary Care Wait times, we worked with SSM Health in their attached primary care clinic to keep their primary care providers at the three provider agreement. EHHS continually works with our two Milton primary care nurse practitioners to use their schedule for maximum patient efficiency as well as conducting numerous pediatric nurse practitioner education sessions (whether in-person or via social media presentations) for the public.For Lack of Alternative Medicine, we continually worked to identify the best strategy in which we might offer the numerous facets of alternative medicine and reached out to a known alternative medicine provider based in Michigan for additional assistance in identification of alternative care.For Housing (senior focus), we met with officials from the City of Edgerton to discuss these needs and potential opportunities in Edgerton and potentially surrounding areas to look at housing opportunities. The City of Edgerton and Edgerton Outreach continued their work to provide low income female veteran housing to assist with the need. We also worked with the Rural Wisconsin Health Cooperative Economic Development staff to further assist with identification of potential housing groups that may be interested in future development.For Senior Health, we provided numerous physician and health provider lunch and learns/education sessions in order to educate patients on services provided and how to obtain these services. We worked on learning more about Chronic Care Management and Transitional Care Management models in order to support the senior healthcare. Discussion also took place regarding hosting an open community health fair in 2025 in order to further educate the public's knowledge of the care available locally. The amounts for community health improvement services and cash for community benefits were determined using the amounts recorded in the general ledger. The cost for subsidized health services reported was determined using the Medicare Cost Report. Part I, Line 7g: The subsidized health services reported on Line 7g includes $948,501 of expenses related to provider based clinics. Part III, Line 2: The amount reported on line 2 represents implicit price concessions. The organization determines its estimate of implicit price concessions based on its historical collection experience with the respective class of patients. Part III, Line 8: Total revenue received from Medicare is the gross reimbursement plus settlement. Both total revenue received from Medicare and the Medicare allowable costs are reported from the Medicare Cost Report. The Medicare Cost Report is completed based on the rules and regulations set forth by the Centers for Medicare and Medicaid Services.Hospital services reimbursed on a fee schedule are not included in the Medicare calculation per the 990 instructions. As part of the Billing and Collection Policy patients are given written notices up to 90 days. A final notice is sent to the patient indicating the account will be sent to a collection agency giving the patient an additional 30 days to respond and complete the Community Care paperwork. After a total of 120 days of notices to the patient, the account is sent to an outside collection agency. For patients known to qualify for charity care or financial assistance, this process is followed for the portion they are responsible for. On accounts which have been assigned to outside collection agencies and are later approved for Community Care, the hospital will request such agencies to cancel and return accounts to the hospital, and to reverse any applicable detrimental credit information created against the patient/guarantor. Part VI, Line 2: We use the following tools to assess health care needs of the communities we serve: (1) Patient exit questionnaires; (2) Involvement in community groups such as the Edgerton Coalition for a Healthy Community where we assess health needs based upon discussions and topics brought up by the Coalition members; (3) Analysis of data received from the Wisconsin Hospital Association and the Rock County Department of Health; and (4) Utilization of the Intellimed software program which allows us to analyze data from facilities comparable to ours.The Community Health Needs Assessment (CHNA) and implementation strategy is complete and posted to our website at www.edgertonhospital.com. Part VI, Line 3: A summary of our community care program is listed on our Hospital website and is contained in a brochure that is available to all patients at all registration areas, emergency room, and waiting rooms in the Hospital. The policy is also provided upon admission and upon request. The complete packet of information, along with an application, is given to any patient upon request. Posters which explain our community care program are located in the Emergency Department and inform patients to contact the Business Office for more information. Billing staff discuss the availability of various government benefits, state programs, and assist the patient on how to qualify for such programs where applicable.
