The Operations manager and the chief operations officer are children of the President and the Executive Vice President. The organization has a program manager who is responsible for affiliate operations. HFUSA has a complete unit that monitors affiliate operations, conducts desk reviews and site visits, and monitors affiliate service delivery. This unit also determines Affiliate grant award amounts, and monitors affiliate service delivery. This unit also has principal liaison responsibility for reporting to HUD, the Grant funds provider. Part VI-Line 11b- A complete copy of the 990 is provided to the Board of Director's meeting next after the completion of the 990. Part VI line 11b- Additionally, the 990 is always on file electronically in the office of the COO of HFUSA. This copy is assessable to all staff and Board Members who request a copy. Form 990, Part VI, Section B, Line 10b Yes HFUSA has a unit that is responsible for the monitoring of Affiliate operations to assure that affiliates are implementing the required procedures of the HUD Grant. These procedures include operational policies, service goals and quotas. These procedures do not completely govern operations, but they do provide guidance and compliance requirements that affiliates must adhere to in conjunction with the HUD grant award. Form 990, Part VI, Section B, Line 11b The 990 is reviewed by the Board and presented to the Board of Directors by the CFO of HFUSA. The 990 is also available by request should a Board member with to review the 990 in detail. The Finance Committee of the Bd has the overall responsibility to review the 990 to determine that it reconciles to the annual audit report, and that any differences are explained to the full Board of Directors. Form 990, Part VI, Section C, Line 19 All documents are available to the public upon request. This is also true for the audit report and management comments. The board is also aware that our audit is published and is public information. Documents and governing documents are available upon request This return is filed late because the original person designated to file the return passed away. As a result of the passing of the return preparer, The Homefree USA audit firm ( Brown & Company) consented to prepare and file the 990, and completed the return and attempted to file the return electronically on 3 separate occasions. (Oct. 13th, 15th and 16th.) Each time the return was rejected by IRS. The rejection was because the EFIN number of Brown & Company was removed by IRS. After many attempts with IRS e services, we could not resolve the EFIN issue. The IRS e services representative, suggested that we try an on-line 990 preparation service. This was November 17th, which only gave us one day to prepare and submit the 990 return. The 990 on-line service required that all data in the return had to be input, we could not transmit a PDF to populate the on-line data entry process. We attempted to put all the information into the return, but we were not able to do so in time to file the return by the November 17th deadline for submission of this 990. We have all the documentation regarding this process, including the death certificate of the original proposed tax return preparer, copies of the three timely filed returns that were rejected by IRS, and we can provide the identification numbers of the IRS representatives that we spoke to in our attempts to complete and timely file this 990 return. We trust that our efforts and the circumstances involving this matter will enable IRS to abate any penalities associated with the late filing of this return.
