Because the Hospital does not pursue collection of amounts determined to qualify as charity care, they are not reported as revenue. The amount of charges foregone for services provided under the Hospital's charity care policy was approximately $98,000 for the year ended December 31, 2024. Additionally, secondary health data from local, state, and national sources was compiled to offer a comparative perspective on health trends and disparities as well as information from the 2021 CHNA.The survey included fifteen (15) questions covering key topics such as health conditions, social determinants of health, and health-related behaviors. Most of the questions were presented in a multi-point scale format, allowing for a deeper understanding of various grouped health categories. The survey was made available to residents in both paper and electronic formats, distributed through White Mountain Regional Medical Center's website and social media platforms, and at various businesses that were willing to retain a sign inviting community members to fill out the survey, ensuring broad reach and accessibility.White Mountain Regional Medical Center included Apache County Public Health from the beginning of the Planning process, through the collection of data and the completion of the assessment. Apache County Public Health was briefed and consulted during the entirety of the process. White Mountain Regional Medical Center Part V, Section B, Line 6b: The community health needs assessment was conducted in collaboration with the Apache County Public Health Services District, Apache County Youth Council, North Country Healthcare, Round Valley School District, Saint John's School District, and Saint John's Community Builders. White Mountain Regional Medical Center Part V, Section B, Line 11: The 2024 CHNA identified 5 health priorities according to the data provided by the community.1) Mental and behavioral Health 2) Alcohol & drugs 3) Women's Health 4) Smoking and Vaping 5) Heart DiseaseAccording to the data reported by the community via the assessment, these are the highest priority needs for the community.Our key focus will be on addressing the needs outlined in the Community Health Needs Assessment, specifically the social determinants of health and the "Big 5" identified in the assessment. Other indicators outlined in the Community Health Needs Assessment such as specialty care access, urgent care services, affordability of healthcare, provider retention and quality of care and healthcare access and infrastructure will be considered based on community needs and the availability of resources and funding.The 2024 CHNA was completed in December 2024. Due to this timing, WMRMC did not have sufficient time to begin addressing the needs identified in the 2024 CHNA. WMRMC took the following actions to meet the needs identified in the 2021 CHNA.1) Access to specialty services:WMRMC contracted with specialists to provide greater access to specialty services in our area. These include Pain Management Providers, Orthopedic Surgeons, Podiatry, contracting with Telemedicine services and home visit services.2) Mental Health Services: Depression, Anxiety, and Substance Abuse:WMRMC contracted with mental health providers to bring these services to our area. We also contracted with providers who had addiction specialties so that we can offer that to our patients. 3) Chronic disease management. WMRMC's utilized its home visit program as a major source of chronic disease management for our area. This service allowed providers to enter patient homes who may have otherwise been unable to see a provider due to chronic disease. White Mountain Regional Medical Center Part V, Section B, Line 13h: Uninsured Patients having annual household incomes of $125,000 or less may, depending upon their assets and liabilities, qualify for discounted pricing for covered services without having to apply for Medicaid assistance. The Hospital determines its estimate of implicit price concessions based on its historical collection experience with this class of patients. Part III, Line 3: The Hospital reasonably estimates the portion of implicit price concessions that could have qualified for charity care. An estimated 7.63% of implicit price concessions in 2024 are attributable to patients potentially eligible for charity care. This is based on the percentage of individuals living below the federal poverty level. Part III, Line 8: White Mountain Regional Medical Center is a critical access hospital. The Hospital provides services to Medicare patients even though there was a shortfall because access to healthcare and individual's health is important to the Hospital and community. Total revenue received from Medicare is the gross reimbursement plus settlement (including LSA). Both the total revenue received from Medicare and the Medicare allowable costs are reported from the Medicare Cost Report. The Medicare Cost Report is completed based on the rules and regulations set forth by Centers for Medicaid and Medicare Services.Hospital services reimbursed on a fee schedule are not included in the Medicare calculation per the 990 instructions. Had this been reported the total loss from Medicaid would have increased by $237,306.Medicare fee schedule revenue: $247,182Medicare estimated costs of care relating to payments (484,488)Net (Shortage) $(237,306) Part III, Line 9b: If a patient qualifies for financial assistance the patient will in no circumstance be charged more than Amounts Generally Billed for emergency services or other medically necessary services. In addition, they will never be required to make advance payment or other payment arrangements in order to receive emergency services. However, they will be required in most situations to make a substantial advance deposit or other payment arrangements based upon an estimate of the Amounts Generally Billed in order to receive non-emergency services. The account will not be turned over to a collection agency while an application is in process. Any personal amount due will follow normal collection procedures which follows a 120-day notification period and 240-day application period, both beginning on the date of the first post-discharge billing statement. The Hospital also provides a 30-day written notice before any extraordinary collection activities. Part VI, Line 2: In addition to the CHNA, White Mountain Regional Medical Center utilizes HealthStream to conduct patient surveys. Part VI, Line 3: The financial assistance policy, application and plain language summary are provided on the Hospital's website in English and Spanish. The forms are also available at the Hospital. Copies of these documents are also available in the Admitting area located near the main entrance of the Hospital and by mail by contacting Patient Financial Services at 928-333-7150. A cover letter is included with billing statements to self-pay patients advising where to learn about government funded programs and an explanation of the financial assistance policy. In addition, flyers with QR codes are present in all registration areas. Part VI, Line 4: White Mountain Regional Medical Center is a 21-bed Critical Access Hospital. The Medical Center serves southern Apache County, Arizona, as well as Catron County, New Mexico. Both are high poverty areas. The population in Springerville, AZ was approximately 2,000. The estimated median household income was $50,134. The poverty rate was 7.63%. The largest industries are retail trade, accommodation & food services, and health care & social assistance. The highest paying industries are utilities, wholesale trade, and construction. The nearest hospital is 45 minutes away in Show Low, Arizona. Part VI, Line 5: The governing body is comprised of persons who reside in the Medical Center's primary service area. White Mountain Regional Medical Center extends medical staff privileges to all qualified physicians. Any surplus funds are used for health education, new services, and equipment and facility upgrades to give our patients the latest in technology.White Mountain Regional Medical Center provides general medical and surgical care for inpatient, outpatient, and emergency room patients, and participates in the Medicare and Medicaid programs.
