The students' academic studies are monitored to ensure compliance with the grant requirements. Pink Ribbon Angels provides assistance to qualified residents of the Wickenburg area to cover underinsured expenses associated with diagnosis of breast cancer. The compensation reported in Part VII represents their total compensation and benefits for calendar year 2024. April Taylor was employed by the Hospital in Volunteer Services prior to becoming Executive Director of the Foundation. Form 990, Part V, Line 2a and 2b: The Executive Directors, Erika Braaksma and April Taylor, were paid by Community Hospital Association Inc, dba Wickenburg Community Hospital, an unrelated organization, for their services to both the Foundation and the Hospital. The Hospital issues the W-2's to report their compensation which is why zero is reported for Part V, Line 2a. Form 990, Part VI, Section A, line 8b There are no committees with the authority to act on behalf of the governing body. Form 990, Part VI, Section B, line 11b The Form 990 is prepared by a third party and is reviewed by the CFO of the Community Hospital Association. A copy of the 990 is made available to the governing board prior to filing with the IRS. Form 990, Part VI, Section B, line 12c All board members and officers are covered under the policy. In connection with any actual or possible conflict of interest, an interested person must disclose the existence of any financial interest and conflict of interest and be given the opportunity to disclose all material facts to the board of directors of the organization. In addition, all interested persons review and sign a compliance certificate annually. After disclosure of any potential conflicts and all material facts, and after any discussion with the interested person, he/she shall leave the governing board meeting and is recused from the discussion and voting on the issue. The remaining board members shall decided if a conflict of interest exists.
