Annually, I will make a full, written disclosure of interests, relationships, and holdings that could potentially result in a conflict of interest." In addition to the statement of intent, board and staff members also complete a questionnaire about fiscal and governance interests related to business and family relationships that could be a conflict of interest. REASON FOR AMENDED RETURN On our originally filed Form 990, the $41,166 dividend income we received in 2023 was mistakenly not included. So our investment asset, total net assets, and dividend income have all been increased by this amount.
