Housing stability is the outcome, and flexible case management is the means to that end. The program finds housing for clients and helps them get food. Once these needs are met, clients are aided to obtain psychiatric medications, substance abuse treatment, and medical care. When some level of stability is reached, volunteering and reconnecting with family is encouraged. Eventually, clients no longer need home visits and graduate to fewer intensive programs, or completely independent living.HPP's Housing First program consists of a Dual Diagnosis Program, Veteran Specific Housing and Colorado Division of Housing case management services. In operation since 2007, the Dual Diagnosis program specializes in assisting the chronically homeless and seriously mentally ill adults with substance abuse issues. The Veteran Specific Housing Program began in 2015 when Rocky Mountain Human Services transferred the grant to HPP. The objective of the program is to provide safe and stable sober living environments, with access to services that support recovery, regaining parental rights if possible, gaining employment, and moving to independent housing. Clients receive intensive case management and are required to attend outpatient treatment within the first 90 days of entering the program.
