Community Hospital's Implementation Strategies: This Implementation Strategy is intended to satisfy each of the applicable requirements set forth in proposed regulations released 2021 and serves as Community Hospital's planned response to the needs identified through 2023 Community Health Needs Assessment (CHNA) process. For a full copy of the CHNA report, please visit: https://health.mesacounty.us/2021chna/ Community Health Needs Identified in the CHNA Report The findings from the 2021 CHNA detail five themes based on Social Determinants of Health (SDH). These Social Determinants of Health: economic stability, education, healthcare and access, neighborhood and built environment, and social and community context. Key Determinants of Health Needs The CHNA broke down the SDH into areas of action and areas of strength. The areas of action include focus areas in each SDH. Household stability including identifying areas in the county that are impacted by stacked disadvantage; and identifying training and educational opportunities for workers transitioning from waning industries, and incentivizing economic growth (Economic Stability). Increasing access to and supporting existing childcare access, focusing elementary proficiency on teacher training and curriculum, support four-year high school completion, and work with those who do not finish graduation. Support higher education students who are the first in their families to seek higher education. Expand concurrent enrollment. Support work of local higher education entities who are creating programs to meet current employer and student demand. Work with employers to provide internships and practical experiences for students (Education). Increase health care access and utilization by investigating root causes for health care use barriers. Research medical debt burden on residents and explore opportunities to increase public insurance plan and subsidy enrollment. Understand current gaps in specialty care. Continue to address the mental health crisis by improving mental health experiences for first-time patients. Reduce barriers to seeking mental health services, and giving consideration to community resources to best keep people in non-emergency substance use intoxication or mental health crises safe while reducing community and health care system disruption (Health Care and Access). Use data to determine the effectiveness of efforts and resources used to affect tobacco, substance use, and obesity interventions. Increase immunization rates in schools. Investigate geography in teen birth rate, and identify services or educational resources that may be lacking in areas with high rates (Health Implications). Access the rates of crime infractions that impact public safety but are not included in violent crime and property crime databases. Develop a clear enforcement process for health and safety violations for Code Compliance and to engage in community clean-up efforts. Explore opportunities to increase food access and determine current gaps in housing assistance and explore housing assistance opportunities (Neighborhood and Built Environment). Survey and focus groups of Mesa County residents to better understand their personal relationships and social networks. Identify strategies to boost social resources in unincorporated areas. Work to address bullying and improve voter participation in low-turnout precincts (Social and Community Context). Health Needs Community Hospital Plans to Address Building on the criteria used in the CHNA process, Community Hospital has evaluated the knowledge, skills, and expertise of our existing assets, and the knowledge of community efforts and entities that are currently, or may be, addressing the identified needs. Further, Community Hospital is leveraging collaborate initiatives that are addressing and impacting the identified needs. Based on this evaluation, Community Hospital has selected a number of needs from the outlined key determinates of health needs and from the broader areas of concern to address. This will allow us to maximize our ability to focus resources and have a meaningful impact on community health needs. Key CHNA data points for each inequity that Community Hospital plans to address are summarized below. 1. Economic Stability Self-sufficiency standard. Data gathered for the CHNA estimates local household expenses based on household makeup-number of adults, as well as the number and age of children. The estimated expenses include housing, child care, food, transportation, health care, taxes, and miscellaneous costs, and accounts for the earned income, child, and child care tax credits. Common household types in Mesa County, which account for 85% of households in the county include Single Female Households with Children, Married Couple Households, and Adults Living Alone. Single female households with children are 6% of the population in Mesa County. The income needed for self-sufficiency for single female with one preschooler is $40,468 and single female with one school-ager and one teenager is $35,619. The Mesa County median income for these households are $19,805 and $19, 805 respectively. The median household income for a single female household with children is approximately half their self-sufficiency income. 20% of households in Mesa County are married couples with children. And additional 33% of households in Mesa County are married couples without children. The self-sufficiency income needed for married couple without children is $32,508. Married couple with one preschooler is $48,268. Married couple with one school-ager and on teenager is $43,038. Mesa County median income for married couple without children is $60,000-75,000. Mesa County median income for married couple with one preschooler is $81,290. Mesa County median income for married couple with one school-ager and one teenager is $81,290. The median household income for married couple households is approximately double their self-sufficiency income. 26% of Mesa County households are adults living alone. The self-sufficiency income for female living alone is $20,470, and the same for males living alone. The Mesa County median income for female living alone is $25,618-26,987 and $27,428-37,250 for male living alone. The median household income for adults living alone is higher than their self-sufficiency income. $50,000 per year is a desirable wage for Mesa County, and $36,400 is the essential income. Of the top ten occupations by number of jobs in Mesa County, Health Care Practitioners and Technical Occupations have an entry-level wage of $43,534 and $70,381 as the median wage. Health Care Support has an entry-level wage of $23,347 and median wage of $27,310. During the COVID-19 pandemic, Community Hospital did not lay off any workers. Recently, robust wages were given to a high percentage of employees, and not a single employee at Community Hospital makes less than $15/hour. 2. Education Focus efforts to improve early childhood outcomes (on children who live in the boundaries of schools with low kindergarten readiness). In 2018, a group of community and business leaders determined that a lack of access to affordable, quality childcare was one of the largest issues facing the community. Mesa County Public Health published Childcare 8000, which attested to the fact that Mesa County is short 4,000 childcare slots. Through internal surveys, Community Hospital found that this need was mirrored in their own employees. The COVID-19 pandemic exacerbated the need for childcare for Community Hospital employees. Over 80% of Community Hospital employees are female, and as essential employees they were required to be at work during the pandemic, when daycare centers and schools were closed. To help serve Community Hospital employees and the greater community, Community Hospital is building a 100 slot childcare center on our main hospital campus. Through multiple RFP processes, Community Hospital has entered into contracts with an architect, operator, and general contractor. The estimated opening date is Fall 2023. Community Hospital continues to work with local and regional universities, and tech schools, to train/educate students seeking careers in healthcare. Further, Community Hospital has been a partner with Mesa County Valley School District 51 to support the district and the community. Community Hospital believes it is important to continue supporting our workforce through educational opportunities. In 2022, Community Hospital received the Pathway to Excellence designation for nursing. We continue to offer programs to the community including bariatric classes, and lifestyle medicine. Many of these offerings were moved online or discontinued during the COVID-19 pandemic, but have been offered once again to help educate our community on the importance of healthcare. 3.
