It provides for the Dr. Louis and Ann Guy Bayside Village Extended Care Corporation services in Baraga County for the treatment and prevention of disabling conditions of its residents. The Grant has provided for the replacement of 5 adjustable and mobile client beds in the 59 bed long-term care and rehab facility. 1 1 Schedule I, Part I, Line 2 All Grantee organizations are required to complete an application for approval by the BCCF Grant Committee and subsequent submittal to the BCCF Board of Directors for approval. All Grantee organizations must complete a Grant Agreement in compliance with the conditions of the grant expenditures. All Grantees must complete a Grant Evaluation form describing expenditures in compliance with the Grant Award and Agreement. Although most Grantees submit photo or other documentation of program completion, completion of programs and projects in the community of Baraga County is always physically evident to the Board and Administration of the Baraga County Community Foundation. Grantees are also requested to present their program information at the Annual Meetings of the BCCF. Form 990, Part VI, Section B, Line 11b The completed Form 990 is on file at the Office where the monthly Board meetings are held and is accessible to all members of the Board for their review at their leisure. Form 990, Part VI, Section B, Line 12c Board Members are fully aware of conflict of interest concerns, and will address any voting decisions in question at the meetings in order to determine a Members eligibility to participate in the vote, or whether they will need to abstain from voting. Form 990, Part VI, Section B, Line 15 A monthly review of job performance is conducted at regularly scheduled Board Meetings, as well as is an annual review of data completed by Board Members and submitted to the Board President who compiles the data for submission to the Board who then addresses it with the employee. Form 990, Part VI, Section C, Line 19 All documents of the Baraga County Community Foundation are available at public request at the Community Foundation Office until another website can be established. The Executive Director of the Baraga County Community Foundation was delayed in the filing of the Form 990 due to the constraints of attending to severe family illness.
