Part VI, Line 11b The form 990 is prepared by the Financial Manager, then reviewed by the Agency Finance Committee and recommended for the approval by the Agency Board of Directors, then it is reviewed and approved for release by the Agency Board of Directors. Part VI, Line 12c At the start of each fiscal year, all members of the Board of Directors sign a statement disclosing any potential conflicts of interest with Commission activities. In addition, the conflict of interest policy is monitored and enforced at each meeting of the Board of Directors to allow self-identification of any conflicts of interest with the agenda scheduled for discussion.
