<ExplanationTxt>Recent information obtained from the 2021 Community Needs Health Assessment (CHNA) continue to guide Sabetha Community Hospital (SCH) energy and resources and have the organization to sharpen it's focus on needs identified. While continuing to build on 2019 data, other areas of unmet needs will become part of the overall strategy moving forward. Access to mental health services continues to be an area of need. This has been previously noted and interaction with local mental health stakeholders has continued. This includes exchanges of information from the local primary care providers and mental health agencies that serve Northeast Kansas. Streamlining referrals and access to these services will continue to be a priority. To that end, plans are underway to facilitate construction or remodeling of a facility that can house and assist providers in delivering the services clients so desperately need. In the interim, ongoing interaction with Rural Health Clinic and hospital staff will continue to be refined. Suicide prevention was identified as an unmet need that will be addressed. This will dovetail off of the mental health service's enhancements. While somewhat of a distinct issue, similar resources can come to bear on addressing this issue. The National Suicide and Crisis Lifeline (988) will serve as a conduit to obtain initial consultations and counseling for clients in need. That cannot, and will not, be the only addition to the suicide prevention activities. Local primary care providers and staff, as well as hospital staff, will continue ongoing education in identification for those in need. Several Sabetha Community Hospital nurses have already taken additional mental health crisis screening courses to begin this endeavor. Another area of unmet need identified in the 2021 CHNA was childcare options. This has certainly been a difficult issue to improve without private sector involvement. That has not deterred Sabetha Community Hospital from pushing forward to plan construction and/or remodeling of a childcare facility that could help to alleviate the local need for this service. While plans are to initially provide this service option for the families of the 150 employees of Sabetha Community Hospital, it would certainly help to alleviate the waiting lists at other local facilities providing childcare. Affordable housing was identified as a need in the area. This has been a longstanding issue without an easy, quick, or single-sided solution. Sabetha Community Hospital has increased wages, especially for the entry level positions, in an attempt to make the living wage amenable to afford decent housing and living conditions. Job description changes for entry level positions have allowed additional hours of employment for workers which has in turn increased overall individual wages for these individuals. SCH will continue to explore creative options for staffing that provides additional opportunities for growth. As has been referenced in prior reports, the Sabetha Health and Wellness Center, completed in July of 2019, has blossomed as an integral part of the community. This facility is managed by SCH. Staffing and programs offered are coordinated through the administration of SCH. The Wellness Center remains very busy with a variety of fitness activities for all ages. Spin classes, aerobic classes, pickleball, basketball leagues, and many other activities continue to grow and provide much needed exercise and fitness options. This was an area identified in the 2021 CHNA and an aggressive stance to educate the community on this facility's activities will continue. Ongoing unmet needs in the area of cancer has been recognized as a need that can be improved. Additional community education has been undertaken to aid in the citizens decisions to be screened, or treated, for cancer at the local level. With the recent addition of a 3D mammography apparatus at SCH there is state of the art breast cancer screening available. Local campaigns to enhance colon cancer screening rates will help the citizens identify risks for colon cancer and screening recommendations. A new surgeon starting at SCH recently has additional training in cancer treatment and screening. This will allow even more patrons to access this service as they will not need to travel 60 miles for this. Youth health education and drug and alcohol abuse were areas identified as needs that have not been immediately addressed. Plans for more integration and coordination into the local school system are underway and at least encouraging. An inpatient drug and alcohol treatment facility is under construction in Atchison which will certainly alleviate on of the bottlenecks in the treatment of individuals in need. Areas previously identified as opportunities for improvement will continue to be addressed. Strengths in the SCH system will continue to be enhanced. Another primary care provider has been signed to a contract and will begin services locally in 2024. Access to care will be improved with new pathways of information for "alternative" forms of insurance. This will include sharing programs, Medicare Advantage, self-pay, etc. Efforts will be made to provide information, assistance, and encouragement to a segment of consumers that have often felt shunned in traditional medical systems. Previous needs assessment information continues to be addressed. Activities that have been ongoing to meet concerns expressed include: 1. Improved equipment and additional staffing for Cardiac Rehabilitation 2. Diabetic education in conjunction with a Registered Dietician 3. Community education regarding low dose CT lung cancer screening 4. Infant car seat safety checks for the community 5. Health publications on social media and local newspaper 6. Sponsoring swimming lesson scholarships for local youth 7. Assistance with prescription drug access for those in need 8. Public and nursing home flu vaccine clinics 9. Ongoing integration with local Emergency Medical Services 10. Providing nursing and healthcare scholarships</ExplanationTxt>

<ExplanationTxt>During 2022, Sabetha Community Hospital continued to monitor through the CHNA Coordinator the three identified Community Health Needs Assessment. Priorities were to promote health, wellness, and chronic disease prevention especially in the areas of hypertension, diabetes, smoking cessation, and increased seatbelt usage to all ages. The hospital continues to actively manage the Health and Wellness Center. Classes are coordinated through employed staff in exercise and fitness regimens. The Hospital Cardiac Rehab Phase III program also closed for several months due to COVID but reopened to provide elders in the community who cannot utilize the Health and Wellness center due to physical limitations, an opportunity for cardiovascular exercises and socialization. The Community Health Needs assessment continues to guide and shape plans for our future. We have met with local mental health providers to gather input and recommendations for assisting them with facilities and coordination of care. Sabetha Community Hospital is a Certified Diabetic Education Facility. The diabetic educator and the dietician teach the SMART program which teaches diabetic patient's accountability for their own care. The hospital conducts low dose CT for lung cancer screening and partners with the local Community Health agency to direct patients to a smoking cessation class. The hospital now employs two certified seatbelt technician who do car seat safety checks for newborn babes leaving the hospital and car seat safety checks in the community as well. Outpatient calendars are sent out monthly to the community and surrounding region. A health scan is done so the health articles are relevant to the current needs of the community. New hospital services are also presented through this publication. That information is posted on the hospital website and Facebook page. Pre-natal classes are conducted quarterly throughout the year for expectant moms and dads. SCH partners with the local health department to provide breastfeeding services through a certified lactation consultant. The breastfeeding consultant provides information through the prenatal classes but then works with moms after baby is born. The two hospitals in the county work together to provide supplies to newborn babes and families. The Level IV Trauma committee remains active in the community by giving scholarships to community children for swimming lessons offered at the public pool. This committee also supports providers of the gun safety workshop. Stop the Bleed instructors from SCH provide e The hospital provides aid to community members who need help enrolling in Medicaid, Medicare, and third-party payers as well as the drug prescription program. This is provided through the hospital social workers and public relations staff. A resource directory is attached to the community health needs assessment on the SCH website. Priority #2 identified improving access to mental health services. The local community mental health center provides access to mental health services through a clinic setting and some telehealth. The hospital does use Avera e-care within the ER setting to help with mental health issues. Resources and the availability of mental health professionals are limited in this rural area.</ExplanationTxt>

Informing patients about the Program in person or during billing and customer service phone contacts. 6. Providing notification of the Program in the patients' monthly billing statement which provides information and resources on how to apply for financial assistance. The Financial Assistance Program takes into consideration a patient's household income and qualifying assets. The Financial Counselor consults with private pay patients to see if they qualify for the Hospital's financial assistance policy. If discovered during the accounts receivable collection process that a patient is having trouble paying bills, both the hospital and the clinic will inform the patient that financial assistance is available. As part of the financial assistance program all applicants must apply for Medicaid. To determine if the patient may qualify for financial assistance, the financial counselor gives patient forms to complete and asks to see bank statements, tax returns, etc. Computations based on current Federal Poverty Level Guidelines are completed to see if qualifications are appropriate and the percentage of charges to be written off is determined, based on family size and household income. All instances of financial assistance are approved by upper management of the Hospital. Information about Financial Assistance is posted on the hospital website and the Financial Assistance form can be downloaded from www.sabethahospital.com. A copy of the FAP Plain Language Summary and an application are provided to patients at discharge and are always available on request from any of the billing and admissions staff. The existence of our assistance policy is posted at main and emergency room entrances, as well as in clinic lobbies.</ExplanationTxt>

<ExplanationTxt>Stop the Bleed instructors conduct classes for the EMS technicians and various local industries. The hospital diabetic educator and dietician conducts the SMART program with diabetic patients which holds them accountable for their diabetic health. Hospital department heads present education on hospital services to the Hospital Guild which meets on a monthly basis. Staff provides presentations for local civic groups to create healthcare awareness on topics such as obesity, stroke/heart attacks, and diabetes. The second largest program Sabetha Community Hospital provides to the community is the Cardiac Rehab Phase III program. Community members can come to the hospital for reduced session fees utilizing cardiopulmonary exercise equipment. The patients who have physical limitations and are not able to use the health and wellness center benefit from the monitoring of this program by an RN or CNA. This program addresses the importance of continual exercise and the health benefits associated but brings with it a social aspect to those community members who participate providing much needed social interaction within the elderly population. In 2022, we served 124 patients in this manner. The third largest program for the hospital is involvement with school-based programs such as clinical rotations for medical students and residents, nurse practitioners, nursing, physical and occupational therapy, radiology, and lab. The hospital is very involved with the local school district hosting students who are interested in healthcare careers and would like to job shadow. The hospital also takes part in the school district school to work program. Students spend part of their day at the hospital or physician clinic as part of their daily curriculum. The hospital also administers six healthcare scholarships to local seniors and community members. The fourth largest program is enrollment assistance for public medical programs such as Medicaid, Medicare, and Medicare Part D. Social Services and Public Relations staff are dedicated to this program during the open enrollment periods. The hospital social worker takes the lead on this project. After the open enrollment periods are closed the Social Worker is still available to the community to help with any assistance needed to for first time enrollees in Medicare or the Medicaid programs. The Social Worker also will assistance community members with needed information and resources, and work with the local pharmacies regarding prescription drug programs if the community member is unable to afford a needed medication. Addressing the needs of mental health among the elderly population as well as youth continues to be a priority. The hospital partners with the regional Community Mental Health Center to provide access to mental health services whether it be in a clinic setting or through telehealth services. Strategic planning for augmenting mental health facilities include 1491 square feet in a hospital addition that will break ground as early as next Spring, 2024. The Community Health Needs Assessment continues to illustrate the importance our families, friends, coworkers and patients place on mental health. We will continue to aggressively pursue this matter. Also outlined as a community need is the existence of a reliable child care facility. Our plan to help address this need include 1399 square feet in the hospital addition that will break ground in Spring of 2024 for a child care facility. Providing stable care with healthy food options for young children is the backbone of any healthy community. Quality healthcare is important to Sabetha Community Hospital and the hospital continues to participate in the Kansas Health Care Collaborative as well as reporting to the Medicare Beneficiary Quality Improvement Program. HCAHPS scores are monitored and used to keep staff informed of patient satisfaction and evaluate areas for needed improvement. SCH's mission remains to provide quality of health and care given in our communities.</ExplanationTxt>
