For air travel, business/first class is allowable for reasons of health (upon determination by Fund management) and for flights exceeding three hours. For rail travel, Fund policy generally requires Coach class be booked. However, in instances where Coach class is not available, Business class is allowable. Schedule J, Part I, Line 3 The Operations & Benefits Committee (the Committee) of the Board of Trustees obtained a salary survey from an independent compensation consulting firm for all employee positions of comparable organizations. The survey was reviewed by the Committee. The survey is also used to support the recommendation of the CEO's salary which is recommended to the full Board of Trustees who have final approval for the proposed salary. For key employee positions, the Committee reviews salary ranges with the CEO who recommends salaries for the Committee to consider. Schedule J, Part I, Line 4 Elliott Buchholz, Joseph DeRocchis and Betty Jo Lesonsky, a former employee, are the only participants in the Fund's 457 deferred compensation plan. Also, employees are given the option to participate in a 403(b) plan that is funded solely through employee salary deferrals. Form 990, Part VI, Section B, Line 11b Management prepares and reviews the Form 990 and then asks McDermott, Will & Emery (Attorneys) to review the draft that is then supplied to each member of the Board of Trustees for their review and approval. Form 990, Part VI, Section B, Line 12c All officers, trustees and key employees are provided with a conflict-of-interest form annually that they are required to complete, sign and submit back to the Fund. Form 990, Part VI, Section B, Line 15 The Operations & Benefits committee of the Board of Trustees periodically obtains a salary survey from an independent consulting firm for all employee positions of comparable organizations. The survey is reviewed by the Operations & Benefits committee. Compensation surveys are also provided to the Board of Trustees. These surveys reflect salary ranges for all staff positions in comparable organizations, as well as staffers' compensation at the time of the survey. Annually, all employees, including the Chief Executive Officer (CEO) complete a performance review. The CEO's salary is reviewed by Operations & Benefits Committee and proposed salary increases are recommended to the full Board of Trustees who have final approval for the proposed salary. Form 990, Part VI, Section C, Line 19 The Fund indicates that governing instruments, the conflict of interest policy, and financial statements are available upon request on our website, in the annual funding notice sent to all participants, and in other various types of correspondence.
